Fertility testing​

 

NEO Fertility offers in-house ultrasounds, laboratory testing, semen analysis, physician and nursing assessments.  

 

Diagnostic Imaging

 

​Transvaginal ​and Transabdominal Ultrasound

A ​transvaginal ultrasound ​(internal) can evaluate the uterus and ovaries. ​We use this test ​to help determine ​your antral follicle count (a marker of ovarian reserve of eggs), as well as diagnose ovarian cysts, fibroids, polyps, or other malformations.​ This test is done in our clinic by one of our fertility trained sonographers.  Most ultrasounds do not require any specific preparation but some will require a full bladder. We will let you know how to prepare. In general, we do not allow a support person at the bedside during the exam. This policy allows us to support our employees by creating a comfortable work environment where they can perform their duties to the best of their abilities​, without distraction and adequate space.​ However we understand there are unique situations so if you feel you are unable to tolerate this exam without a support person, please contact the clinic immediately to discuss your situation and help us work with you towards a solution.  

 

Saline Infusion Sonohysterogram (SIS)

This specialized transvaginal ultrasound procedure is performed by one of our on-site gynecologists. This test is used to evaluate the inside of the uterus and the functionality of the fallopian tubes. Sterile saline fluid is infused inside the uterus via a thin catheter, allowing your physician to visualize the uterine shape and cavity, while assessing the patency (openness) of your fallopian tubes. This test requires the use of a speculum at the beginning, which will be carefully positioned by your gynecologist. In general, no preparation is necessary for this procedure but some patients will be recommended to take antibiotics. Your bladder should be empty for this test. 

 

Laboratory Testing

 

Hormone Blood Tests

These blood tests evaluate various hormones (FSH, LH, Estradiol, ​Progesterone​, PRL, TSH, DHEAS, Testosterone) to assess both egg quantity and quality. A combination of hormone blood tests (AMH, Day 3 FSH) and ​transvaginal ultrasound evaluates your ovarian reserve. We can use this information to see if the egg count is in a normal range for your age​ and it also helps predict how you may respond to various fertility ​medications. Some patients will also be recommended to include AMH testing. AMH testing is not covered by OHIP insurance. 

 

Semen Analysis

NEO Fertility has in-house Medical Laboratory Technologists (MLT) to perform semen analysis and sperm washes. This test is required to assess the overall health of the sperm. The sperm sample test evaluates semen volume, sperm concentration (number of sperm), motility (swimming ability), morphology (sperm shape), and if there are any infections present. We do require an appointment for this procedure to ensure one of our MLTs is ready to process your sample immediately for optimal accuracy. 
 

Intrauterine Inseminations (IUI) 

 

IUI is a fertility treatment where specially prepared sperm are placed directly into a woman's uterus around the time she is ovulating. The goal is to increase the chances of the sperm reaching and fertilizing an egg.

 

The basic steps usually include:

  • Monitoring the woman’s cycle (sometimes using medication to boost egg production)

  • Collecting and "washing" the sperm (to select the best ones)

  • Using a thin catheter to insert the sperm directly into the uterus.

 

IUI is less invasive and less expensive compared to IVF, and it's often one of the first treatments recommended for couples with unexplained infertility, mild male factor infertility, or cervical issues. 

 

In Vitro Fertilization (IVF)

 

IVF is a type of fertility treatment where an egg and sperm are combined outside the body, in a lab, to create an embryo. Then, a doctor places one (or more) of those embryos into the woman’s uterus, hoping it will implant and lead to a pregnancy.

 

The basic steps are:

  • Ovarian stimulation – The woman takes medications to make her ovaries produce multiple eggs 

  • Egg retrieval – A doctor collects the eggs through a minor procedure.

  • Fertilization – The eggs are mixed with sperm in a lab (or sperm is injected directly into the egg).

  • Embryo culture – The fertilized eggs grow in the lab for a few days.

  • Embryo transfer – One or more embryos are placed into the uterus.

NEO Fertility can do Satellite monitoring with any IVF clinic to allow you to have all or the majority of your ovarian stimulation appointments closer to home. The egg retrieval, fertilization, embryo culture and transfer are done at the IVF clinic, under the direction of your Reproductive Endocrinology and Infertility specialist.

 

If the embryo implants, it leads to pregnancy! NEO can then carefully monitor the initial pregnancy with blood work and ultrasounds until 11 weeks. 

 

IVF is often used when other treatments haven’t worked, for older women, or in cases like blocked fallopian tubes, severe male infertility, or certain genetic conditions.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 


 

Ovulation Induction 

Ovulation induction is a fertility treatment where medications are used to help a woman’s ovaries produce and release eggs.

Normally, a woman releases one egg per month. With ovulation induction, the goal is to either:

  • Help women who aren’t ovulating at all (like in PCOS), or

  • Help produce more than one egg to increase the chances of pregnancy.
     

How it usually works:

  • The woman takes oral medications (like Clomid or Letrozole) or hormone injections (like FSH).

  • Doctors monitor the growth of the egg follicles using blood tests and ultrasounds.

  • When the eggs are ready, another medication (a "trigger shot") may be given to release the eggs for timed intercourse, IUI, or even IVF.

Donor Eggs & Surrogacy 

 

Surrogacy is when another woman — called a surrogate or gestational carrier — carries and delivers a baby for someone else.
It’s used when someone (like a woman with medical issues or no uterus) can’t safely carry a pregnancy themselves.

The surrogate does not use her own egg. Instead, an embryo created using the intended parents’ egg and sperm (or donor egg/sperm) is placed into her uterus.
 

Egg donation is when a woman donates her eggs to help someone else have a baby.

It’s used in fertility treatments when:

  • A woman’s eggs are not healthy enough

  • She’s older and has fewer good-quality eggs

  • She has a genetic condition she doesn’t want to pass on
     

How it works:

  • A donor takes medication to stimulate her ovaries to produce multiple eggs.

  • The eggs are retrieved, fertilized with sperm (from the intended father or donor sperm), and embryos are made.

  • One embryo is placed into the intended mother’s or surrogate’s uterus.

 

 

 

 

Oncofertility Preservation and Egg Freezing

 

Oncofertility Preservation (for cancer patients)

When someone is diagnosed with cancer, treatments like chemotherapy and radiation can harm fertility.
Oncofertility preservation means saving eggs, sperm, or embryos before cancer treatment starts.

For women, the most common option is egg freezing or embryo freezing.


For men, it's usually sperm freezing.

The goal is to give cancer patients a chance to have biological children after they survive and recover.

Timing is important — the preservation needs to happen quickly, usually before starting chemotherapy or surgery.

NEO Fertility works with oncologists and IVF clinics to offer these services. Typically patients are seen within 24-48 hours. 

 

Egg Freezing (also called oocyte cryopreservation)

Egg freezing is a process where a woman’s eggs are collected, frozen, and stored for future use.

How it works:

  • A woman takes hormone medications to stimulate her ovaries to produce multiple eggs.

  • The eggs are retrieved through a small procedure.

  • Instead of fertilizing them, the eggs are frozen and kept in a special storage facility.

  • When she’s ready later (months or even years), the eggs can be thawed, fertilized with sperm, and used to try to achieve pregnancy.

It’s commonly used by:

  • Women who want to delay pregnancy for personal or career reasons

  • Women concerned about declining fertility with age
